The Nurse Practitioner (NP) provides advanced clinical expertise in geriatrics and palliative care as a key member of Sharp Memorial Hospital’s Generational Health Program. This program is dedicated to improving the care of older adults by supporting function, dignity, independence, and quality of life throughout the healthcare continuum. Through a collaborative, interdisciplinary approach, the NP partners with multiple inpatient service lines, physicians and surgeons, nursing, pharmacy, case management and social work, therapy and rehabilitation, specialty services, and community resources to develop individualized, patient-centered care plans that align with each patient's goals, values, and clinical needs.
