Health RIGHT 360, a network of programs across California, seeks a Case Management professional to support the STOP Area Six initiative in the San Diego region. You will manage a caseload of CDCR clients, develop CMPs, and coordinate transitions from custody to community-based care.
You will also arrange placements, monitor progress, document thoroughly in electronic systems, and collaborate with parole and community providers to connect clients with needed services.
