Position Summary

The Community Mental Health Therapist is an important member of the YMCA’s interdisciplinary care team, serving residents in San Francisco ’s public housing communities – Alice Griffith, Sunnydale, Potrero Hill, and Hunters View – in collaboration with the San Francisco Department of Public Health (SFDPH) and the Mayor’s Office of Housing and Community Development (MOHCD).

This role provides direct mental health services, conducts clinical assessments, and engages in community outreach with youth and families. The therapist works collaboratively with a Health Care Provider, CHAMPS (Community Health Access & Mobilization for Prevention and Support) team members, and a Care Coordinator to deliver comprehensive, culturally responsive care. The position supports the City’s community wellness initiatives by integrating mental health services within public housing communities, promoting resilience, and increasing access to care.

Job Responsibilities Clinical Services
  • Conduct assessments and develop individualized, trauma-informed, and culturally responsive treatment plans.
  • Provide individual, family, and group therapy for youth and adults referred through SFDPH, YMCA programs, and partner agencies.
  • Collaborate with Intake Coordinators to ensure timely connection to services.
  • Maintain accurate and compliant documentation in line with YMCA, SFDPH, Medi-Cal, and MOHCD requirements.
  • Participate in interdisciplinary case consultations and monitor client progress toward treatment goals.
Community Engagement and Outreach
  • Conduct outreach in public housing communities to promote mental health awareness and access to care.
  • Co-facilitate community-based workshops and events on emotional wellness, stress management, and family support.
  • Build collaborative relationships with residents and community partners to strengthen trust and inclusion.
  • Represent the YMCA at community meetings and wellness initiatives.
Interdisciplinary Collaboration
  • Work within an integrated care team alongside healthcare providers, CHAMPS staff, and Care Coordinators.
  • Participate in regular case conferences, coordination meetings, and team trainings.
  • Support data collection and reporting to demonstrate community impact and inform public health planning.
Administrative and Compliance
  • Complete required documentation (e.g., progress notes, treatment plans, discharge summaries) in compliance with standards.
  • Participate in supervision, professional development, and required trainings.
  • Maintain confidentiality in accordance with HIPAA, FERPA, and local privacy laws.
  • Contribute to quality assurance, program evaluation, and chart review activities.
Qualifications
  • Master’s degree (MA/MS) in Counseling, Clinical Psychology, Marriage and Family Therapy, or Social Work, or within one academic year of completion.
