SUMMARY OF POSITION

The LCSW will provide assessment, diagnosis, treatment, crisis intervention, and case management for members with mild to moderate behavioral health needs; and assessment, diagnosis, crisis intervention, brief treatment and referral of members with acute or chronic psychiatric and/or behavioral health needs. The LCSW will provide direct social work care management services to PACE participants and works collaboratively with the PACE Interdisciplinary Team (IDT) to manage long-term care needs.

The LCSW will also supervise ACSWs.

Essential Job Functions
  • Conducts psychosocial assessments to determine PACE Participants’ care needs, preferences, and goals.
  • Ensure compliance of PACE Social Worker staff through supervision in all social work areas including documentation, assessments, case management, psychotherapy, and care plan completion.
  • Develops treatment care plans and interventions and continues to engage in participants to improve behavioral health conditions.
  • Participates as an active member of the IDT for Participant care planning and communicates effectively and openly with other Interdisciplinary Team members on participant status and needs.
  • Provides appropriate crises intervention/treatment to participants in emergent situations including assessment, counseling, information/referral and providing consultation to PCP and IDT team.
  • Performs roles and responsibilities of PACE Social Worker (MSW and ACSW).
  • Documents accurately and thoroughly in the Participants’ electronic health record in accordance with industry and departmental standards including time requirements.
  • Develops and lead group counseling, support groups, and/or other relevant activities.
  • Plans, develops, implements and coordinates a clinical treatment program for the assessment, diagnosis, treatment, crisis intervention, case management for members with behavioral health needs; and assessment, diagnosis, crisis intervention, brief treatment and referral of patients with acute or chronic psychiatric and/or behavioral health needs.
  • May provide after-hours on-call coverage to support PACE participants and address their behavioral health needs when the PACE Center is closed as needed for support.
  • Under supervision and in conjunction with the medical director, develops, implements, and leads both the quality improvement and outcome evaluation programs for behavioral health services.
  • Provides care to Participants in-person, via telehealth, and/or via telephone, as indicated.
  • Utilizes Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
  • Performs other job duties as required by manager/supervisor.
Qualifications Required
  • Master’s degree in Social Work from an accredited college or university required.
  • Active LCSW in the State of California for at least 2 years out of the last 5 years.
  • Has practiced psychotherapy at least 2 years out of the last 5 years.
  • Supervisors are required to have completed 15 hours of training, stay current with ongoing annual trainings, and need to submit documentation to Board of Behavioral Sciences.
  • Must have already completed and submitted a Supervisor Self-Assessment Report to the Board of Behavioral Sciences.
  • Current BLS and First Aid required.
Preferred
  • Minimum 2 years of experience in a health-related area required.
  • Minimum one year of experience working with a frail and/or older adult population preferred.
  • Prior experience directly supervising ACSWs.
  • Valid California Driver’s license with good driving history within the last 5 years.
Language
  • Must be able to fluently speak, read and write English.
  • Fluency in Chinese (Cantonese and/or Mandarin) is preferred.
  • Fluency in other languages is an asset.
Status

This is an FLSA exempt position. This is not an OSHA high-risk position.
