Position Summary:
The TAY HWC Case Manager outreaches to, intakes, assesses, counsels, and coordinates care for TAY ages 18-27 years old to achieve safety, health, and well-being across multiple life domains such as housing/shelter, education and employment, behavioral health, physical health, community building, family and relationships, and much more. The Case Manager serves as the client’s main advocate and care coordinator within GLIDE’s continuum of care, community partners and resources, and other external systems of care.
This position works closely and collaboratively with other TAY HWC team members to enable warm hand-offs and linkages amongst specialists, case managers, navigators, and ambassadors. Upon exit from GLIDE’s care, the Case Manager develops a care transition plan to ensure the clients are linked to additional support services and knows how to reconnect with GLIDE for future support. In addition to 1:1 case management support, this position will also facilitate and/or coordinate weekly activities or psychoeducation groups that will support, promote, and enhance clients’ confidence, wellness, community connection, and growth mindset.
Work schedules may include the need to work on one day, during the weekend. Working hours may also vary and occur during nontraditional hours.
Essential Duties and Responsibilities:- Initiate and conduct outreach with TAY at the TAY HWC Center and at offsite locations where TAY may access or congregate for population engagement
- Administer intake and assessments to enroll TAY into GLIDE’s continuum of care, establish baseline of safety and health needs, and develop individual goal/service plans that include actionable steps for care coordination.
- Develop a care coordination plan based on 1:1 session with client, referral information, and/or assessment and identify key care coordination needs within GLIDE’s integrated services (e.g. Health Access, Family Resource Center, Meals, etc.) including GLIDE partners (e.g. UCSF, Street Medicine, Clean Slate, Larkin St.) and establish successful linkages to care
- Support TAY to navigate and overcome barriers related safety, health, and economic stability by teaching and coaching TAY on how to self-advocate complex service systems using effective communication tools and strategies
- Identify personal knowledge and skills gaps that may negatively affect TAY ongoing stability and success and link TAY to educational resources to increase essential knowledge and skills, including but not limited to sexual health education, personal hygiene, pet care, harm reduction principles, building healthy relationships, adult literacy, digital literacy, interfacing with banking institutions, etc.
- Coordinate, facilitate, and/or directly conduct expressive arts activities, therapeutic activities or psychoeducation groups that will support, promote, and enhance clients’ confidence, wellness, community connection, and growth mindset.
- Monitor client progress through regular scheduled appointments and follow-up to ensure successful implementation of individual service plan and maintain accurate, timely, and up-to-date records of TAY intake, assessments, progress, and outcomes in required databases and hard copy files as required
