We are seeking a compassionate, organized, and team-oriented Per Diem Licensed Speech Therapist (ST) for Santa Clara County to provide high-quality home health care.
Responsibilities- Work closely with a multidisciplinary team to ensure coordinated patient care.
- Deliver speech therapy services per individualized care plans and evaluate patient progress.
- Maintain accurate clinical documentation and follow company policies.
- Collaborate with physicians, case managers, and nursing staff to support patient outcomes.

Requirements- Current California Speech Therapist license.
