Role
Summary:
In this role, you will be a key member of the care team, delivering personalized nursing care, coordinating with healthcare professionals, and promoting overall patient health and wellness. This is an exciting opportunity to make a meaningful difference in patients’ lives while growing your clinical expertise.
Duties / Responsibilities:- Provide high-quality, patient-centered nursing care, including assessment, planning, implementation, and evaluation.
- Administer medications, treatments, and procedures according to established protocols and patient care plans.
- Monitor patients’ conditions, document findings, and report changes to the care team promptly.
- Educate patients and their families on disease management, health promotion, and preventive care.
- Collaborate with the care team, physicians, and other healthcare professionals to coordinate and optimize patient care.
- Assist in the development and implementation of individualized care plans for patients with complex needs.
- Maintain accurate and timely patient records, ensuring compliance with all regulatory and organizational requirements.
- Provide support for both in-person and telehealth visits, as applicable.
- Ensure adherence to infection control, safety, and HIPAA regulations.
- Participate in continuing education and professional development opportunities.
- Perform other related duties as assigned.
- Active Registered Nurse (RN) license in CA.
- Bachelor’s or Associate’s Degree in Nursing from an accredited program.
- Strong clinical, assessment, and patient care skills.
- Excellent communication, interpersonal, and patient education skills.
- Ability and willingness to travel to patient locations, if required.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications, if applicable.
- Valid driver’s license.
Qualifications:
- Experience in home health, community care, or outpatient nursing.
- Ability to work collaboratively within a multidisciplinary care team.
- Strong commitment to patient advocacy and health promotion.
- Comfort with electronic health records and telehealth technologies.
