Certification Details

• BLS - Basic Life Support
• ACLS - Advanced Cardiac Life Support

• Responsible for Care Coordination and Care Transitions Planning throughout the acute care patient experience.
• Works in collaboration with the Physician, Utilization Manager, Medical Social Worker and bedside RN.
• Assures the timely progression and transition of patients to the appropriate level of care to prevent unnecessary admissions or readmissions.
• Facilitates care across the continuum through effective resource coordination.

• CA RN Licensure
• 2 years of Case Management experience
• Utilization Management experience in working with attending physicians supporting level of care protocols

• 5 x 8 Days
• Weekend rotation / call as needed
