Job Details

Conduct Medicare annual health assessments
See 5-8 patients a day
Perform H&Ps and document in EMR systems
Collaborate with patients' PCPs to ensure continuity of care

Requirements
Active NP license in California
Bilingual (Cantonese, Mandarin or Vietnamese required)
