Pay Rate Range$35-45/hr*
* Rate dependent on experience, skill level and case assigned

The Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under the supervision of a Registered Nurse in accordance with the care plan and physicians’ orders, maintaining confidentiality and professional boundaries at all times.
Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient conditions, reporting changes to the supervisor or physician.
- Document all activities, assessments, nursing actions, responses and coordination of care in a timely manner.
- Participate, implement and update the nursing care plan.
- Take appropriate nursing action based on assessment to achieve expected outcomes.
- Recognize changes in patient needs and respond with interventions to prevent or reduce risk.
- Accept responsibility for personal and professional accountability by complying with company policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.
- Provide care utilizing infection control measures that protect both staff and patients according to OSHA standards.
- Educate patients and families regarding disease processes, self‑care techniques, and prevention strategies.
- Maintain knowledge of competencies through educational programs, CE units and internal training.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care policies and procedures.
- Must have reliable transportation.
- Continuing education as required by state.
- Minimum one year of hands‑on nursing experience (CA LVN license required).
- Current CPR certification with hands‑on component.
- TB skin test (2‑step) current within last 12 months.
- South Carolina – One (1) year of pediatrics experience.
- California – One (1) year of experience working under current nursing license.
- Louisiana – One (1) year of experience working as a licensed nurse.
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Attention to detail.
- Time management.
- Effective problem‑solving and conflict resolution.
- Good organization and communication skills.
- Speak, write, read and understand English.
- Travel as required.
- Lift 50 pounds.
- Reposition patients and move equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Sit and climb stairs.
- Visual and hearing acuity.
- Strong sense of smell and touch.
- Respond physically and mentally to emergency situations in the home or during transport.
- Wide variety of environments, possibly exposing to allergens and other conditions.
- Possible exposure to blood, bodily fluids and infectious diseases.
