Since 1984, Larkin Street Youth Services has helped more than 75,000 young adults in San Francisco with outreach, shelter, housing, education, employment, healthcare, and other services. Today, Larkin Street Youth Services is the largest provider of housing and services to homeless youth in the Bay Area, serving 1,500 youth annually across multiple sites in San Francisco.

WHAT YOU'LL DO

Under the direction and guidance of the Program Manager, you will provide comprehensive, strength‑based case management services to homeless and/or marginally housed transitional aged youth (ages 18‑24). You will conduct a comprehensive evaluation of bi‑psychosocial, emotional, educational, vocational and functional needs, develop individualized service plans, and leverage community resources as needed. The Case Manager II leads collaborative service planning, must have a thorough understanding of the stages of change and development, and will provide services outside normal office settings, including emergency shelters, congregate living housing, scattered sites, and SRO placements throughout the city of San Francisco.

  • Conduct intake and assessment; direct service linkage and referral; case planning and contracting; case coordination and advocacy; follow‑up and after‑care.
  • Develop and maintain strong relationships with clients.
  • Provide crisis prevention, intervention, and safety planning services, along with stress‑management and life‑skills development.
  • Assist young people in applying for and obtaining benefits and housing.
  • Provide client resources from outside agencies and supportive services that promote health and wellness.
  • Work closely with community behavioral health providers and internal service teams to ensure coordinated and effective services.
  • Maintain accurate, detailed observation and progress records; complete necessary electronic data entry into agency and city databases; identify trends in data to assist in service planning.
  • Adhere to confidentiality laws (HIPAA) and reporting requirements; maintain compliance documentation.
  • Collaborate with staff members and established community partners, including property managers and landlords.
  • Facilitate transportation and provide accompaniment to client appointments in the community.
  • Complete safety and wellness checks regularly in residential units.
  • Teach life skills (e.g., cooking, cleaning, budgeting).
  • Participate in organizational meetings (e.g., all‑staff, CQI, Client Practice, etc.).
  • Provide services when and where clients need them, including evenings and weekends when program needs dictate.
  • Support program services within the milieu, e.g., crisis intervention or maintaining the safety of the program.
  • Other general duties contributing to the care and welfare of the clients we serve.
LOCATIONS
  • Residential Programs
    • Location:

      Any of our housing programs located throughout San Francisco (including sites in the Tenderloin neighborhood).
    • Hours:

      9 AM to 5 PM or 10 AM to 6 PM, depending on site needs.
    • Working Days:
      Tuesday through Saturday or Sunday through Thursday (including weekends).
  • Drop‑In Center
    • Location:

      The Engagement and Community Center (ECC), 134 Golden Gate Ave, San Francisco, CA.
    • Hours:

      9 AM to 5 PM or 8 AM to 4 PM, depending on site needs.
    • Working Days:
      Monday through Friday.
