Bayview Hunters Point with medical and behavioral health services that encourage them to make decisions that support their health, safety, and development. 3rd Street Youth Center & Clinic also manages a number of successful Transitional Age Youth (TAY) housing stabilization programs and one of the city's few Youth Access Points.

TAYwill assistclientswithsecuringneededdocumentationto providebarrierremovalassistancerelatedtohousing, employment etc.
Theywillalsoprovidewrap-aroundcasemanagementservices withaharmreductionand trauma informed model to ensure the clients long-term self-sufficiency and improved well-being. The Case Manager - Barrier Removal for TAY will work closely alongside the clients care team.

-Barrier Removal for TAY . Duties, responsibilities, and activities may change, or new ones may be assigned as needed.
- Create Individual Service Plan (ISP) and attendance log (ex: Housing, Mental Health, Work, Relationships)
- Connect youth to appropriate wrap-around services available to them as well as provide warm hand-offs to wrap-around providers.
- Resource brokerage and referral as needed to community resources for behavioral health, employment, education,families, public benefits, legal, low-income housing, In Home Support services (IHSS), and etc.
- Case coordination and advocacy within agency and with other Community Based Organization’sCBO's.
- Connect youth to longer-term support through 3rd Street or another agency.
- Facilitate/coordinate youth in-person contact with a referral identified on their Service Plan (ISP).
- Assist youth with achieving at least 50% of their ISP goals through case management as evidenced by form Individual Service Plan (ISP) review.
- Create housing retention plans surrounding self-sufficiency .
- Conducting service assessments, including designing and conducting pre-and-post surveys.
- Continually provide youth with weekly case management services
- Data collection and case notes entered into all appropriate databases.
- Attend community, city, & regional housing meetings.
This role requires a strong focus on administrative tasks to support housing services, including maintaining detailed client records,managing physical files, completing extensive documentation, and ensuring accurate data entry in systems such as HMIS/GMS for program compliance and reporting.
Candidates should be comfortable handling a high volume of paperwork, organizing both digital and physical records, collaborating withcase managers, engaging directly with clients when needed, and adhering to organizational and funding requirements to best support our clients' housing stability.
The ideal candidate will possess strong organizational skills, attention to detail, and the ability to manage multiple deadlines.
Skills, Knowledge, and Abilities (SKA):- Ability to perform tasks and responsibilities in a complete and timely manner, complying with agency policies and standards and conforming to the requirements of this job description.
- Ability to work with a diverse staff and excel in a multicultural environment.
- Ability to work with minimal to moderate supervision.
- Ability to provide referrals/linkages for substance abuse disorders or behavioral health disorders for counseling and notice symptoms of use and abuse, recovery, and treatment philosophies.
- Proficiency in data management systems, Microsoft Office (especially Excel and Word), and effective file organization is essential.
- Strong communication skills, including the ability to navigate conflict, lead with compassion, and maintain appropriate professional boundaries.
