Initiatives such as the Youth Outreach Squad and Health Core empower Transitional Age Youth (TAY) with education, wellness resources, and career preparation, fostering a more equitable society where youth from all neighborhoods can lead fulfilling lives.
Role DescriptionThis is a full-time on-site role located in the San Francisco Bay Area. As a Case Manager, you will provide client-centered, strengths-based support to Transitional Age Youth (TAY) ages 18‑27, helping them secure stability in housing, healthcare, education, and employment. Your day‑to‑day responsibilities include assessing client needs, developing individualized service plans, coordinating access to resources, managing case files, and advocating for clients within social service systems.
You will also support youth in reaching personal goals and navigating pathways to self‑sufficiency through mentorship and empowerment.
- Strong case management and advocacy skills, with an ability to assess, plan, and execute individualized service strategies
- Ability to connect clients with housing resources, healthcare, education, and workforce development programs
- Excellent communication, relationship‑building, and interpersonal skills, especially with Transitional Age Youth (TAY) and diverse communities
- High adaptability, organizational skills, and effective time management, with the capacity to handle multiple cases and administrative tasks
- Knowledge of social services systems, trauma‑informed care, mental health support, and community resources
- Proficiency in cultural competency, particularly in addressing the unique challenges faced by historically underserved communities
- Prior experience in social work, counseling, or a related field is highly beneficial
- Bachelor’s degree in Social Work, Psychology, or a related field preferred; equivalent experience and demonstrated skills will be considered
