We are a compassionate, patient‑centred hospice and home health agency dedicated to providing exceptional end‑of‑life and skilled care services to patients and families across San Mateo, Santa Clara, Contra Costa, Alameda, and San Francisco counties. Our interdisciplinary team focuses on dignity, integrity, and building strong relationships with referral partners, community providers, patients, and their families.

With a role centered on building and maintaining referral relationships with physicians, hospitals, skilled nursing facilities, assisted‑living communities, and community partners, you will drive census growth, educate the community on hospice and home‑health services, and represent our agency with professionalism and heart.

Qualifications

• Develop and maintain strong referral relationships with hospitals, SNFs, ALFs, physicians, case managers, discharge planners, and community partners.
• Conduct in‑service presentations and educational events on hospice and home‑health services.
• Identify new business opportunities and strategic partnerships.
• Maintain a consistent presence in the community through face‑to‑face visits and outreach.
• Track and report referral activity, market trends, and growth metrics.
• Collaborate closely with intake, admissions, clinical leadership, and executive teams.
• Ensure all marketing activities comply with Medicare, state, and ethical guidelines.
