Location: Santa Clara County – Campbell, Cupertino, Gilroy, Los Altos, Los Altos Hills, Los Gatos, Milpitas, Monte Sereno, Morgan Hill, Mountain View, Palo Alto, San Jose, Santa Clara, Saratoga, Sunnyvale.
Qualifications:
- LVN/LPN licensed to practice in the state and in good standing.
- Graduate of an approved school of practical/vocational nursing.
- One (1) year experience as a professional nurse in an acute setting.
- Current CPR certification (BLS required).
- Valid driver’s license.
- Valid automobile insured per LORIAN requirements.
- Ability to read and follow written instructions and document care in the patient’s electronic medical record.
- Excellent observation, nursing judgment, and oral and written communication skills.
- Self‑directing and able to work with little supervision.
- Capable of working with adults (18+) demonstrating tact and patience.
- Flexible and cooperative in fulfilling role obligations.
- Participates in in‑service education programs and clinical meetings.
- Adheres to infection‑control policies and protocols to protect patients and employees.
- Maintains confidential patient records in accordance with HIPAA and applicable state laws.
Responsibilities:
- Provide direct nursing care to clients as prescribed by the physician, under the direction and supervision of the Registered Nurse (RN).
- Perform nursing activities in compliance with the State LVN Practice Act, licensure requirements, and Lorian Health policies.
- Travel to patients’ homes on a daily basis and document all care electronically via a smartphone provided by Lorian Health.
- Observe patients regularly for changes in condition, discuss changes with patients, families, or caregivers, and report and document changes as necessary to the attending physician.
- Educate and instruct patients, families, and caregivers regarding patient care.
- Assist with other duties as assigned that are related to job responsibilities.
Compensation: Pay Per Visit model – $30/visit to $40/visit, commensurate with experience and visit type.
