Compensation & Core Requirements
  • Pay Rate: $30‑35 per hour
  • Minimum 1 year of hands‑on nursing experience required; new graduates not accepted
  • Must possess reliable transportation and maintain an active, unencumbered CA LVN license
  • Current CPR certification with hands‑on component
  • TB skin test – 2‑step recent within last 12 months

Qualifications
  • Active, unaffiliated California LVN license
  • Current CPR certification
  • TB skin test – 2‑step (certified within last 12 months)
  • One year prior hands‑on nursing experience
  • Reliable transportation
  • Proficiency in clinical assessments, documentation, and compliance with nursing policies and procedures
  • Continuing education as required by state
Position Overview

The Licensed Practical Nurse (LPN/LVN) provides skilled nursing care and documentation under the supervision of a Registered Nurse. Responsibilities include creating and managing patient care plans, improving patient outcomes, and ensuring confidentiality and professional standards at all times.

Essential Job Functions
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition, reporting changes to supervisor and/or physician.
  • Document all activities, assessments, nursing actions, and care coordination promptly.
  • Participate in nursing care plan implementation and updates.
  • Take appropriate nursing actions based on assessment results.
  • Recognize changes in patient needs and intervene to prevent or reduce risk.
  • Maintain accountability by complying with policies, regulations, accrediting bodies, and the Nurse Practice Act.
  • Use infection control measures per OSHA standards.
  • Educate patients and families on disease processes, self‑care, and prevention.
  • Maintain knowledge of nursing competencies through continued education and internal programs.
Additional Requirements
  • Graduate of an accredited school of nursing.
  • Unrestricted state license as a licensed nurse in the state of practice.
  • Current CPR certification.
  • Proficiency in assessment, documentation, and nursing policy compliance.
State‑Specific Requirements
  • South Carolina – One (1) year of pediatrics experience.
  • California – One (1) year of experience working under current nursing license.
  • Louisiana – One (1) year of experience as a licensed nurse.
  • Continuing education as required by state.
Preferences
  • Six (6) months of recent experience as a licensed nurse in a clinical care setting.
  • Home health experience.

Physical & Environmental Requirements
  • Fluency in English (speaking, writing, reading).
    Travel required.
    Lifting ability: 50 pounds.
    Ability to reposition patients and move equipment unaided.
    Prolonged walking, standing, bending, kneeling, reaching, twisting.
    Ability to sit, climb stairs, adequate visual/hearing acuity, sense of smell and touch.
    Physical response capability for emergency situations.
  • Function in diverse environments; possible exposure to allergens, blood, bodily fluids, infectious diseases.
