The Certified Nursing Assistant / Home Health Aide (CNA / HHA) position works under the supervision of a Case Manager / DON and is responsible for providing resident care to assigned residents according to assessments, care plans, and directives from nursing management. The CNA / HHA collaborates with licensed care staff to coordinate appropriate care types and strictly follows all company nursing policies, procedures, and safety guidelines.

• Strong commitment to improving quality of life and partnering with community organizations in patient care
• Ability to work well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and STNA leadership
• Eagerness to pursue higher certifications and further education opportunities
• Ability to work closely with patients on a 1-on-1 basis while receiving strong support from a patient care team
• Ability to work independently on a day-to-day basis
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance
• Certified Nursing Assistant (CNA) certification in the state(s) of practice
• Home Health Aide (HHA) certification in the state(s) of practice
• CPR certification
