Position: HOME HEALTH REGISTERED NURSE

Overview

Outstanding Registered Nurses needed for field-based positions servicing multiple Santa Clara County locations:

ACE Home Health and Hospice is a locally owned and operated agency with offices in Orinda and San Jose, CA. We are growing and seeking talented and compassionate professionals to join our team. Positions can start as per diem and could grow into full-time employment. Position title:

Registered Nurse - Home Health

Responsibilities
  • Assess and plan care for home health patients in accordance with the RN scope of practice and CMS guidelines.
  • Create OASIS/start of care documentation, conduct follow-up visits after start of care, and coordinate care.
  • Manage a team of therapists to ensure patients meet home health goals and can be discharged back to the community with an excellent patient experience.
  • Provide care coordination and ensure high-quality patient outcomes.
Qualifications
  • 1 year of full-time experience in Med/Surg or an acute setting; at least 1 year of home health experience.
  • RN unrestricted license in California; CPR certification;
    California Driver’s License with valid auto insurance.
  • Skilled in IV infusion, phlebotomy, wound care including wound vacs, NGT/GT, and teaching and management of disease processes.
  • Ability to manage an independent schedule; supervise LVNs and CHHAs; strong communication, integrity, and loyalty.
  • Flexibility with Bay Area travel between homes.
  • Experience with home health software and other electronic communication.
  • Critical thinking, time management, and attendance at case conferences/staff meetings.
  • Ability to work one weekend per month is a plus.
