Guardian Angel Home Care is seeking a compassionate, dependable, and experienced
Licensed Vocational Nurse (LVN) to provide
private duty nursing care for a patient in the San Jose, California area
.

This position offers a consistent weekday schedule, working
Monday through Friday from 11:00 AM to 5:00 PM
, providing dedicated one‑on‑one nursing care in the comfort of the patient's home.

We are looking for an LVN who is passionate about patient care, comfortable working independently, and committed to building a positive relationship with the patient and their family.

Competitive hourly pay: $45–$50
, depending on experience

11:00 AM – 5:00 PM (30 hours per week)

Supportive clinical leadership and administrative team

Electronic documentation through Homecare Homebase (HCHB)

Opportunity to provide personalized, meaningful patient care

Provide dedicated one‑on‑one skilled nursing care to a patient in their home.

Monitor the patient's condition, vital signs, and overall health status.

Administer medications and treatments as prescribed and within the LVN scope of practice.

Assist with activities of daily living and patient care needs as appropriate.

Observe and promptly report changes in the patient's condition to the supervising RN and physician.

Maintain accurate and timely clinical documentation.

Communicate professionally with the patient, family, and interdisciplinary care team.

Follow the established plan of care and all applicable agency and state regulations.

Maintain a safe, comfortable, and supportive environment for the patient.

Active
California Licensed Vocational Nurse (LVN) license in good standing

Graduate of an accredited vocational nursing program

Current BLS/CPR certification

Strong communication and patient care skills

Ability to work independently in a private home setting

Dependable and available for the assigned weekday schedule

Previous private duty nursing or home health experience

Experience providing one‑on‑one patient care

Familiarity with electronic medical documentation systems

Strong medication administration and patient assessment skills
