Job Summary:While maintaining good public relations and implementing the goals and vision of the regional center, this position is responsible for providing and coordinating services to adult individuals in accordance with the provisions of the Lanterman Developmental Disabilities Services Act, and policies established by the San Andreas Regional Center Board of Directors. The Adult Service Coordinator’s primary role is ensuring that individuals have access to services that enable them to reside in the least restrictive community setting consistent with their choices and abilities.
Essential Job Functions:- Conduct required visits and meetings to gather data and produce initial and/or ongoing assessments of needs for services
- Coordinate the Person-Centered Planning process with individuals and their families
- Develop the Individual Program Plan (IPP)
- Counsel individuals concerning identification and achievement of personal goals
- Advocate with individuals/families to assure provision of needed services
- Initiate and follow up on referrals to service providers and community resources
- Identify areas of unmet needs
- Perform tasks related to fiscal and accountability requirements
- Complete all required caseload documentation
- Develop expertise in accessing generic resources
- Assure that all individuals and families are informed verbally and in writing of their right to appeal whenever a decision is made to change, alter, or deny a service
- Know Title 17 and Title 22 licensing and DDS requirements
- Know all resources that serve our individuals who reside in various counties
- Intervene effectively in crisis situations to eventually effect stability
- Monitor residential care facilities in compliance with Title 17 regulations
- Perform annual and quarterly visits as required
- Monitor Day Programs and other adult services (Independent Living Skills/ILS), Supported Living Services/SLS, employment services, and housing services to assure quality provision of services
- Evaluate conservatorship requests and submit reports to the appropriate parties as designated
- Service Coordinators are mandated reporters for both CPS and APS
- Serve as liaison to service providers including care homes, day programs, and other community agencies
- Have court involvement on a case-by-case basis, which includes attending, participating, and providing progress reports to the courts
- Handle fiscal responsibilities such as social security benefits in collaboration with fiscal department
- Oversee judicial involvement cases
- Collaborate with several agencies to ensure seamless accountability of service delivery
- Conduct required home visits and meetings in various settings
