About Life Moves
Life Moves is the largest and most effective provider of housing and services for neighbors experiencing homelessness in Silicon Valley since 1987. Our Mission is to end homelessness by providing interim housing, supportive services, and building collaborative partnerships. With 40 programs, Life Moves gives our neighbors experiencing homelessness a temporary place to call home while providing intensive, customized case management through site‑based programs and community outreach.
PositionPurpose
SHIFT - Sunday‑Thursday 8:00 AM–4:30 PM. The Branham Lane Interim Supportive Housing (ISH) Case Manager reports to the site Program Director and coordinates supportive services for 204 clients, including housing, employment, benefits, physical and behavioral health. The role encompasses case management, daily site operations, client support, group facilitation, crisis intervention, documentation, and community collaboration.
Key Responsibilities- Develop and maintain individualized case plans focused on housing, employment, benefits, and overall well‑being.
- Support clients with housing searches, employment readiness, benefit enrollment, budgeting, and daily living skills.
- Connect clients to healthcare providers, community resources, and coordinate scheduling and transportation.
- Maintain accurate documentation and outcome tracking in HMIS and the agency’s client management system.
- Advocate for client needs and rights when working with partners, landlords, employers, or service agencies.
- Provide calm, trauma‑informed crisis response and align services with ethical standards.
- Build and maintain partnerships with community organizations, public agencies, and system partners.
- Participate in weekly case conferences, monthly trainings, and regular supervision.
- Support daily site operations, including inspections, maintenance coordination, donation management, and facility upkeep.
- Facilitate group sessions or workshops that promote skill development and confidence.
- Be available to work occasional evenings and weekends as needed.

- Attend continuing education opportunities.
- Be available to work evenings and weekends as needed.
- Operations and Site Responsibilities
- Support daily site operations.
- Maintain clean, safe, organized living environments.
- Facilitate group sessions or workshops.
- Educational:
Associate’s Degree in related field preferred; or High School Diploma/GED with 2–5 years of equivalent experience. Related fields may include peer advocacy, community outreach, housing services. - Technology:
Comfortable using phone and messaging systems, email, Microsoft and Google Suites, databases for administrative tasks and case management.
