Role Description

This is a part‑time, on‑site role for a Harm Reduction Specialist, based in San Jose, CA. The Harm Reductionpiar will provide support and resources to individuals facing challenges related to substance use, housing instability, and mental health. Responsibilities include performing client assessments, developing intervention strategies, offering referrals and education, collaborating with partner agencies, advocating for client needs, and delivering harm reduction services.

Documentation of client interactions and participation in relevant team meetings are also key aspects of the role.

Qualifications
  • Experience in case management, client assessments, and the development of harm reduction strategies
  • Strong interpersonal and communication skills to engage effectively with individuals from diverse backgrounds
  • Understanding of harm reduction principles, supportive housing programs, and mental health resources
  • Ability to work collaboratively with partner agencies and community organizations
  • Proficiency in maintaining accurate documentation and utilizing case management software
  • Bachelor’s degree in Social Work or related human services field preferred
  • Knowledge of the challenges faced by homeless or at‑risk populations in the Bay Area
  • Empathetic approach to working with clients facing complex needs
  • Multilingual ability is a plus
  • Valid driver's license and reliable transportation
