Home Health Licensed Vocational Nurse (LVN) Orange County

We are looking for an energetic Licensed Vocational Nurse (LVN) who will travel to patients' homes throughout the day and provide direct nursing care under the supervision of a Registered Nurse. All documentation will be done through an electronic device (smartphone) provided by Lorian Health.
Key Responsibilities- Travel to patients' homes and provide nursing care as prescribed.
- Observe patients for changes in condition, discuss findings with patients or their family/caregivers, and document and report changes to the attending physician as required.
- Educate and instruct patients, families, and other caregivers regarding patient care.
- Adhere to infection‑control policies and protocols.
- Maintain and securely store confidential patient records in compliance with HIPAA and applicable state laws.
- LVN/LPN licensed to practice in the state and in good standing.
- Graduate of an approved school of practical/vocational nursing.
- One (1) year experience as a professional nurse in an acute setting.
- Possesses and maintains current CPR certification (BLS required).
- Possesses and maintains a valid driver’s license.
- Possesses and maintains a vehicle insured per Lorian Health requirements.
- Ability to read and follow written instructions and document care in the patient’s electronic medical record.
- Possesses excellent observation, good nursing judgment, and good oral and written communication skills.
- Is self‑directing and can work with minimal supervision.
- Ability to work with adults (18 years and older) with tact and patience.
- Flexible and cooperative in fulfilling role obligations.
- Participates in in‑service education programs and clinical meetings.
- Adheres to infection‑control policies and protocols.
- Maintains confidential patient records in compliance with HIPAA and applicable state laws.
We are a Pay Per Visit compensation model. Pay Per Visit rates range from $30/visit to $40/visit, commensurate with experience and varying by visit type.
