Help Launch a New PACE ProgramThis separate business entity helps older adults with complex medical and social needs remain safe, independent, and connected to their communities.
The organization is seeking a mission-driven Geriatric Services Manager to lead and help build the social work function for its new PACE center. This is a unique opportunity to join before the first participant is enrolled and help shape the care model, team culture, workflows, and participant support structure from the ground up.

The Geriatric Services Manager will report to the Center Director and work closely with the interdisciplinary team, PACE leadership, and community partners.
