Overview

Life Long Medical Care is an amazing opportunity for a highly motivated Registered Nurse II to join our multidisciplinary team providing exceptional primary care services at our East Oakland Health Center in Oakland and our Eden PATH location in San Leandro. This position will split time between both locations.

Under the general supervision of the Supervising Registered Nurse RN IV and Center Director, the Registered Nurse II is responsible for providing clinical care and working to integrate whole‑person care into our new team model. The registered nurse will participate in telephonic and in person patient triage, education and counseling, and will collaborate with medical providers to ensure optimal health of our patient populations.

The ideal candidate is committed to providing high‑quality primary care for complex patients, dedicated to building a collaborative team model of care, and has experience working with behavioral health diagnoses. We offer ongoing education and collaboration with medical and behavioral health providers to ensure optimal health outcomes.

This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain in good standing as defined in the CBA.

Benefits

Compensation: $51.00 to $59.13 per hour, depending on years licensed, with an additional 3% pay increase for candidates with a Master’s degree. Benefits include medical, dental, vision (including dependent and domestic partner coverage), generous leave benefits with nine paid holidays, Flexible Spending Accounts, 403(b) retirement savings plan, and commuter benefits.

Responsibilities
  • Coordinate team‑based care by focusing on the intersection between medical and social needs of our patients.
  • Act as a resource for support staff for triaging walk‑in and telephonic care; play a role in the new patient intake process, working closely with primary care providers to ensure complete evaluation of patient needs.
  • Facilitate chronic disease management activities.
  • Coordinate primary care clinic services with other providers and agencies.
  • Coordinate with Care Transitions team for all hospitalized patients to ensure timely follow‑up and return to primary care.
  • Use clinician‑approved standing orders to order labs and preventive screenings, and to refill routine medications.
  • Coordinate care closely with other team members to identify patient self‑care goals and to assure that medical and psychosocial needs are met.
  • Work as an essential team member in the Integrated Medical and Mental Health collaboration with Alameda County case‑management partners.
  • Commit to the provision of services for the underserved with demonstrated ability and sensitivity in working with a variety of low‑income populations, diverse education, lifestyle, ethnic and cultural origins.
