Job Summary
We are a premier home health provider committed to innovative, compassionate care with superior outcomes. As a Home Health RN, you develop patient care plans in collaboration with patients, caregivers, physicians, and other health professionals. You will conduct comprehensive assessments in patients’ homes, provide skilled nursing care, and monitor patients’ conditions on a regular basis.

• Perform comprehensive assessments in patients’ homes and identify their needs.
• Develop a care plan in collaboration with patient/caregivers, healthcare providers, case managers, and supervisors; document appropriately.
• Assess the need for additional services and equipment on admission and inform the case manager or supervisor in a timely manner.
• Assess the home environment and make recommendations to enhance patient care, promote safety, and prevent adverse events.
• Take verbal orders from providers promptly and document them accurately.
• Initiate appropriate preventive, rehabilitative, therapeutic, comfort, and pain interventions, providing instructions to patients, caregivers, and staff promptly.
• Demonstrate professionalism by maintaining high clinical standards and staying current on medication and nursing issues.
• Update clinical records, including medication profiles, care plans, OASIS, case conferences, progress notes, and re‑certification documentation as required.
• Serve as a preceptor when needed.
• Perform other duties as assigned.

• Graduation from an accredited nursing program.
• Current state license as a Registered Nurse and CPR certification.
• At least one year of clinical experience outside of school/rotations; home health experience preferred.
• Must have a vehicle, valid driver’s license, and vehicle insurance.
• Strong interpersonal and communication skills.
• Ability to make home visits and provide skilled nursing assessments and care in the home.
• Experience with HCHB documentation preferred.
