Registered Nurse (RN) - Home Health, San Luis Obispo County
Full-time | Pay: $55.00 - $65.00 per hour | Associate's degree required | Current RN license and CPR certification | Must have a valid driver's license and vehicle.
Position Summary
We are a premier home health provider in our community, enhancing care experience for patients, families and referral sources. As a Home Health RN, you will develop and implement care plans, conduct patient assessments, and collaborate with interdisciplinary teams to ensure high-quality, compassionate care.
Responsibilities
- Perform comprehensive assessments in patients' homes and identify needs.
- Develop care plans in collaboration with patients, caregivers, providers, and case managers; document accurately.
- Assess and indicate need for additional services or equipment on admission and ongoing basis.
- Assess home environment and recommend improvements for safety and care.
- Take verbal orders from healthcare provider promptly.
- Initiate and implement preventive, rehabilitative, therapeutic, comfort, and pain interventions; provide instructions to patients/caregivers and staff.
- Maintain professionalism and stay current on clinical and medication issues.
- Maintain clinical records including medication profiles, OASIS, progress notes, re-certification documentation.
- Serve as preceptor when needed.
- Other duties as assigned.
Qualifications
- Graduation from an accredited nursing program.
- Current state RN license and CPR certification.
- At least one year of practical nursing experience outside of school; home health preferred but not required.
- Must have vehicle, driver's license, and auto insurance.
- Excellent interpersonal and communication skills.
- Ability to visit homes and provide skilled nursing assessments and care.
- Experience with Home Health and Home Care Home Base documentation preferred.
