Salary: $55.00 - 65.00 per hour

King City, CA (must live within a 10‑mile radius of King City or Paso)
Join a premier home health provider that delivers innovative, compassionate care. As a Home Health RN you will develop and implement individualized care plans in collaboration with patients, families, and physicians. You will travel to patient homes to assess conditions, administer treatments, and monitor progress, ensuring high‑quality nursing care in a home setting.
Responsibilities- Perform comprehensive assessments in patients’ homes and identify patient needs.
- Develop a care plan in collaboration with patients, caregivers, and healthcare providers, documenting appropriately.
- Assess and recommend additional services and equipment on admission and as needed, informing the case manager or supervisor.
- Evaluate the home environment to recommend safety improvements and prevent adverse events.
- Take verbal orders from healthcare providers promptly and document accurately.
- Initiate preventive, rehabilitative, therapeutic, comfort, and pain interventions, providing instructions to patients, caregivers, and staff.
- Maintain high clinical standards and stay current on medication and clinical practice issues.
- Update clinical records, including medication profiles, OASIS, case conferences, progress notes, and re‑certification documentation.
- Serve as a preceptor when needed.
- Other duties as assigned.
- Graduation from an accredited nursing program.
- Current state license as a Registered Nurse and CPR certification.
- At least one year of clinical practical nursing experience outside of school or rotations; home health experience preferred.
- Valid driver’s license, reliable vehicle, and automobile insurance.
- Strong interpersonal and communication skills.
- Ability to provide skilled nursing assessments and care during home visits.
- Experience with HCHB documentation preferred but not required.
- Excellent pay and 23 days of PTO per year.
