Purpose and Scope
Outpatient Registered Nurse (RN) is an entry level designation within the Clinical Advancement Program (CAP). The RN provides and coordinates clinically competent care including assessment, planning, intervention, and evaluation for an assigned group of patients. The RN may delegate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians). As part of the End Stage Kidney Disease (ESKD) health care team the RN participates in decision‑making, teaching, leadership functions, and quality improvement activities.
PrincipalDuties and Responsibilities
- Perform essential functions under the direction of the Supervisor and guidance of the Clinical Educator, Preceptor, or other RNs.
- Collect and analyze patient data pre‑during‑post hemodialysis treatment, document in the patient medical record, adjust treatment plan, and notify Team Leader, Charge Nurse, Supervisor or Physician as needed.
- Assess, collaborate, and document patient/family learning needs, and provide initial and ongoing education.
- Direct and collaborate with direct and ancillary patient care staff to safely deliver hemodialysis therapy to assigned patients.
- Administer medications as prescribed or per approved algorithms and document justification and effectiveness.
- Initiate or assist with emergency response measures.
- Serve as a resource for the health care team, participate in staff training and orientation of new staff.
- Ensure correct laboratory collection, processing and shipping procedures and reschedule missed or insufficient collections.
- Document and update nursing assessment and plan of care with the Interdisciplinary Team.
- Ensure patient awareness of transplant and treatment modality options.
- Complete CAP requirements to advance.
- Perform all other duties as assigned by Supervisor.
- Regularly involve heavy lifting, moving of patients, and assisting with ambulation.
- Frequent, prolonged periods of standing; must bend over.
- May move machines and equipment up to 200 lbs and lift materials up to 30 lbs at 5 ft.
- Work environment characteristic of a health care facility with air temperature control and moderate noise levels.
- May be exposed to infectious and contagious diseases/materials.
- Day‑to‑day work includes desk work, computer work, interaction with patients, facility/hospital staff and physicians.
- May require travel to training sites or other facilities.
- May provide essential functions in other locations with same physical demands.
- Supervise Patient Care Technicians/LPNs/LVNs/RNs as Team Leader or designated Nurse in Charge after meeting all criteria:
- Successful completion of all FKC education and training requirements.
- Minimum 9 months RN experience.
- Minimum 3 months chronic/acute hemodialysis experience.
- Graduate of an accredited School of Nursing.
- Current state licensure (RN).
- Current or successful completion of CPR/ BLS Certification.
- Meet state practice requirements.
- Entry‑level RNs with less than 2 years of nephrology nursing experience.
- Chronic/acute hemodialysis experience preferred.
