Overview

This is a full‑time position Monday through Friday, 8:00 a.m. – 5:00 p.m. with no weekends, on‑call, or major holidays. The role can be a Level 1, Level 2, or Senior level based on candidate experience.

Responsibilities
  • Plan, implement, coordinate, evaluate, and promote the continuity of care consistent with established standards and practices.
  • Assess health status by interviewing patient regarding current status and medical history.
  • Assess patient/family needs and provide teaching and other instructional material.
  • Provide psychosocial support to patients and families.
  • Facilitate clinical operations and patient care.
  • Triage patient phone calls as needed and document relevant information in the patient's chart.
  • Facilitate communication between patient/family and interdisciplinary team and coordinate services related to patient care.
  • Assist patients in locating available community resources.
  • Act as a resource to LPN/LVNs and medical assistants.
Qualifications
  • Level 1 – Graduate from an accredited program for professional nursing education, BSN preferred; RN state license and current registration;
    Current Basic Life Support (BLS) certification; 0‑3 years nursing experience, preferably at least one year in oncology.
  • Level 2 (in addition to Level
    1)
    – 3‑5 years nursing experience including at least one year in oncology.
  • Senior (in addition to Levels 1 and
    2)
    – Five years nursing experience, including at least one year in oncology.

Physical Requirements

Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye‑hand coordination. Requires standing and walking for extensive periods of time. Occasionally lifts and carries items weighing up to 40 lbs. Requires corrected vision and hearing to normal range. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
