Job DescriptionA leading home health organization in San Marcos, California is seeking an RN Case Manager to join its growing clinical team!
Pay: $55.00 - $65.00/hour
- Weekend and after-hours on-call rotations
- Comprehensive benefits package including medical, dental, vision, retirement benefits, and paid time off
- Complete patient assessments and determine home care needs
- Develop and revise individualized plans of care
- Administer medications and treatments as ordered by physicians
- Educate patients and families regarding treatment plans and healthcare needs
- Coordinate care with physicians and interdisciplinary healthcare teams
Qualifications:
- Minimum of one year of recent acute care experience required
- Current BLS/CPR certification required
- Valid driver's license and auto insurance required
- Home health experience preferred
- Serves patients throughout Carmel Valley, San Marcos, Carlsbad, and surrounding communities
- Minutes from beautiful Southern California beaches and outdoor recreation
