Requirements:

• Current LVN license in the State of California
• One year of experience preferred
• Experience in hospice preferred

Duties Include:

• Servicing a range of patients and families during end-of-life transitions
• Ensuring quality and safe delivery of hospice services
• Implementing current nursing practices and providing direct, hands-on care within scope of practice
• Obtaining orders as needed to promote comfort and quality of life
• Keeping the Case Manager informed of all changes in condition and orders
