Physical Therapist (PT) Level 3—Home Health—San Mateo Peninsula, CA. Full‑time or per‑dime, with a $10,000 sign‑on bonus and competitive hourly pay ($81–$91/h) and annual salary $170,000–$190,000. This is not a contracted position.

• Dedicated home health‑experienced clinical team to support you in the field—whether assisting with a new situation, contacting a patient if you’re running late, or following up on lab results.

• Provide physical therapy services within the scope of practice, following individualized plans of care.
• Deliver treatment, evaluate patient response, and report changes to the physician, case manager, and/or director of nursing.
• Maintain appropriate clinical records and submit documentation on company devices per organizational policy.
• Follow all company policies and procedures.
• Perform other duties as assigned.

• Current California PT license (required).
• Valid California driver’s license.
• Minimum 2 years of home‑health experience; proficiency in OASIS and case management.
• Minimum 4 years of home‑health experience; expertise in complex case management; clinical specialization a plus.
• Coverage area: regional floater; willing to cover wider service area based on priority patient needs.
• Schedule:
  
  high availability; willingness to work weekends or urgent coverage shifts as needed.
• PTA: substantial PTA supervision.
• Ability to travel within the San Francisco area and surrounding regions.
• Caring, compassionate personality with strong communication skills.

Full‑Time / Per Diem.

Flexible. Monday‑Friday standard, with rotational weekend coverage (1 weekend per month/quarter) required.

Hourly: $81.00 – $91.00 per hour. Annual expectation for full‑time: $170,000 – $190,000 per year + $10,000 sign‑on bonus.
