American Traveler is hiring an experienced RN Case Manager for an acute care hospital setting requiring 2 years of direct case management experience and an active CA RN license.

• Acute hospital Case Management department
• Day shift, 5x8 hours, 8:30 AM to 5:00 PM
• 13-week assignment
• Patient ratio of 1:20
• Complex patient population with chronic or complicated medical conditions

• Active CA RN license required at time of submission
• BLS certification required
• ACM and Interqual certifications preferred
• Minimum 2 years of direct acute care Case Management experience required
• Any charting system experience accepted
• Community hospital experience preferred
• 2 supervisor references preferred

• Conduct initial patient assessments and ongoing stay reviews
• Collaborate with interdisciplinary team to evaluate patient needs and create comprehensive discharge plans
• Coordinate discharge to appropriate level of care for complex patients
• Must reside within 75 miles of the facility
• RTO requests always require facility approval and must be submitted upfront
• First-time travelers not considered for this role
