Life Moves is seeking a Bayside HOT Case Manager to support neighbors moving toward safe, stable housing. You will provide trauma-informed case management, life skills coaching, and resource connections in partnership with clients to set goals and overcome challenges.
You will build trust through empathetic, client-centered support, coordinate housing and benefits access, and collaborate with community partners to strengthen services while documenting progress in HMIS and internal systems.
This is an excellent position to take on the Trauma-Informed Outreach Case Manager role at Life Moves.
