Job Overview
We are looking for an Associate Practitioner to join our Community Nursing Team on the Isle of Wight. To work as part of the interdisciplinary team to provide comprehensive physical therapy to avoid hospital admission.
Responsibilities- Assist in identifying health and social needs of patients and participate in the delivery and evaluation of care provided under the indirect supervision of qualified staff (Registered Nurses, Physiotherapists and Occupational Therapists).
- Ensure high standards are maintained in a safe environment, promoting equality and sensitivity for all individuals.
- Provide assessment and prescription of programs spanning Physiotherapy, Occupational therapy and Nursing, including optimising mobility, re‑training of personal and domestic activities of daily living, building confidence and providing appropriate multi‑disciplinary input and guidance.
- Excellent interpersonal skills and a passion for learning and development in the field of therapies aimed at avoiding hospital admission.
- Demonstrated flexibility, reliability and excellent organisational skills.
- Highly motivated, able to work as part of a dynamic and developing integrated team providing holistic, person‑centred care across a variety of community settings.
