We are looking for a dedicated Physical Therapy Assistant to work in a home health setting. You will provide quality care under the supervision of a Physical Therapist.

• Under the supervision of a Physical Therapist, provide physical therapy services, re‑assess the patient’s status on each visit, and ensure quality of care as documented in the clinical record.
• Accurately administer standardized tests, treatment procedures and modalities as assigned by therapists.
• Assist therapists in developing or modifying goals and objectives of patient care based on assessment data.
• Implement appropriate treatment strategies, procedures, and modalities according to the plan of care.
• Communicate patient progress, status and response to therapy services to the supervising therapist, patient, family and interdisciplinary team.
• Provide a safe, adequate and comfortable environment for the delivery of therapy services.
• Serve adult patients in the routine performance of duties.

• Graduate of an approved Associate Degree program as a PTA and licensed in the state of Ohio.
• One to two years of experience in a hospital or rehabilitation facility preferred.
• Demonstrated ability to keep abreast of new research and practices in the field to provide the best possible care and treatment.
• Effective written and oral communication skills.
• Ability to lift patients as necessary and be mobile.
• Valid State of Ohio motor vehicle operator’s license and insurable under Firelands’ auto policy; proof of personal automobile liability insurance coverage upon request.
• Reliable punctual attendance at assigned work location.
• Ability to work safely, maintain an accident‑free workplace, and demonstrate working knowledge of emergency codes.
• Compliance with OSHA regulations, CDC standards and transmission‑based precaution recommendations, and proper use of PPE; knowledge of S.D.S. forms.
• Successful completion of a ninety‑day probationary period.
