• Treat patients according to the treatment plan of care developed under the supervision of the Licensed Physical Therapist.
• Communicate patient progress, problems, and plans with the Physical Therapist, patient/family, and facility staff.
• Participate in Patient Plan of Care Meetings and Interdisciplinary Team Meetings as requested.
• Assist in in-service training and education for facility staff.
• Record treatment notes, progress notes, and discharge summaries; complete daily billing of services per policy and procedure.
• Instruct families and nursing staff in maintenance programs under the supervision of the Physical Therapist.
• Participate in discharge planning under the supervision of the Physical Therapist.
• Achieve or exceed the minimum productivity standard established by CRI.
• Recommend equipment to facilitate improved delivery of clinical services.
• Report equipment problems to appropriate personnel.
• Act as clinical trainer for affiliated Physical Therapist Assistant programs.
• Comply with Physical Therapist Assistant Licensure and Certification Standards.
• Work beyond usual hours as needed for emergencies, weekends, and holidays.
• Consistently portray the mission, vision, core values, and professional image of Concept Rehab.
• Perform special projects and other duties as assigned.

• Associate’s Degree in Physical Therapist Assistant.
• Licensed as a Certified Physical Therapist Assistant in the state(s) where services are provided.
• Strong relationship and interpersonal skills; high emotional intelligence.
• Outstanding oral and written communication skills.
• Problem‑solving, analytical, and critical evaluation skills.
• Technology skills and cultural awareness.
• Ethical practice and ability to exercise independent judgment and discretion.
• Maintain confidentiality of patient information.
