- Education: Graduate of an accredited School of Advanced Practice Nursing
- Licensure: Current Ohio Nurse Practitioner license
- Certification: Board Certified as a Family Nurse Practitioner
- Experience:
- 3+ years of clinical experience as a Nurse Practitioner
- Experience in community health, ambulatory care, or FQHC settings
- Soft Skills: Compassionate care, team collaboration, patient education, quality improvement focus
- Deliver comprehensive primary care services, including patient exams, diagnosis, and treatment.
- Develop and manage individualized care plans, referring patients to specialists as appropriate.
- Collaborate with an integrated care model, partnering with behavioral health and care coordination teams.
- Prescribe medications in accordance with Ohio laws and the FNP scope of practice.
- Provide patient education for preventive care, chronic disease management, and overall wellness.
- Participate in quality improvement and team-based initiatives to enhance clinical outcomes.
- Support clinical team members while modeling compassionate, high-quality care.
