About Seva Senior CenterSenior Social Center is a licensed Adult Day Health Care (ADHC) program in Irvine, CA, dedicated to serving seniors with dignity, clinical excellence, and culturally compassionate care. Our interdisciplinary team focuses on improving participants’ health outcomes while supporting families and caregivers.
Position SummaryWe are seeking a California-licensed Registered Nurse (RN) to join our ADHC clinical team. The RN plays a key role in participant assessments, care planning, clinical oversight, and coordination with physicians and interdisciplinary team members to ensure high-quality, compliant, and compassionate care.
Key Responsibilities- Conduct initial, monthly, quarterly, and nursing assessments
- Develop, implement, and update Individualized Care Plans (ICP)
- Monitor participants’ health status, vitals, and chronic conditions
- Coordinate care with physicians, families, therapists, and social services
- Administer medications and treatments per physician orders
- Maintain accurate and compliant documentation per ADHC / CBAS regulations
- Participate in interdisciplinary team (IDT) meetings
- Identify changes in condition and initiate appropriate clinical interventions
- Ensure compliance with Title 22, Medi-Cal, and ADHC regulations
- Educate participants and families on health management and wellness
- Assist with infection control, emergency preparedness, and quality assurance activities
- Active California Registered Nurse (RN) license (in good standing)
- Minimum 1 year RN experience (ADHC, geriatrics, home health, or long-term care preferred)
- Knowledge of ADHC / CBAS regulations a strong plus
- Experience working with seniors and chronic conditions
- Strong assessment, documentation, and care‑coordination skills
- Excellent communication and teamwork abilities
- CPR/BLS certification (or willingness to obtain)
- Ability to work in a multidisciplinary healthcare setting
- Experience in Adult Day Health Care (ADHC) or CBAS
- Familiarity with Medi-Cal documentation and audits
- Bilingual (Gujarati / Hindi / Spanish) – desired but not required
- Experience with culturally diverse senior populations
