Active Home Health, Hospice & Personal Care is looking for a Full Time Home Health RN to care for our patients in SLC County. • Visit and assess patients to complete an initial comprehensive OASIS or assessment and ongoing assessments through end of care.
• Chart all clinical documentation in our EHR software.
• Create and manage patient plan of care.
• Educate and give instruction to patient, family, and caregivers as appropriate per patient care plan.
• Case manage the patient episode or benefit period, ensuring that all cares are performed and documented.
• Provide medication education and management per physician order.
• Work hand‑in‑hand with the IDT to ensure patient goals are met and appropriate outcomes are achieved.
• Share on‑call duties.
• Maintain strong communication with patient, family, caregivers, and interdisciplinary teams.

• Registered nurse with current license to practice professional nursing in the state of Utah.
• Polite and compassionate.
• Excellent observation, problem solving, verbal and written communication skills.
• Maintain a current CPR certification.
• 1 year Home Health or Hospice experience preferred.
• Must be a licensed driver with an automobile that is insured, registered and in good working order.
• Good communication and smartphone skills.
• Ability to pass a background check.
