Field LPN - Home Health Visits

Orange, Seminole & Osceola Counties, FL | Part-Time / Per Visit

• Deliver care in accordance with the patient s individualized Plan of Care under RN supervision.
• Provide prompt communication of changes in patient condition/emergencies to the team.
• Perform skilled nursing tasks within LPN scope of practice, which may include:

• Medication administration
• Wound care
• Infusion therapy
• Transferring & positioning patients
• Catheter care

• Current LPN license in the State of Florida (required)
• IV Certification (required)
• Strong wound care skills
• 1+ year of home health experience preferred
• Excellent communication, teamwork, and collaboration skills
• Proficiency with electronic health records (EHR)
