The Occupational Therapist (OT) in Home Health provides skilled therapy services to patients in their homes to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities. The OT works with patients recovering from illness, injury, or living with chronic conditions to help them perform activities of daily living (ADLs) safely and independently.

- Evaluate patients' functional status, home environment, and therapy needs.
- Develop individualized treatment plans based on evaluation findings and physician orders.
- Implement therapy interventions to improve patients’ ability to perform self-care, work, and leisure activities.
- Educate patients and caregivers on techniques to improve function and safety.
- Recommend and train patients in the use of adaptive equipment as needed.
- Document clinical notes, evaluations, treatment goals, and progress reports accurately and timely.
- Collaborate with the interdisciplinary care team to ensure comprehensive patient care.
- Comply with all federal, state, and local regulatory requirements and company policies.
- Graduate of an accredited Occupational Therapy program.
- Current Occupational Therapy license in the state of practice.
- Minimum of one year of OT experience preferred (home health experience is a plus).
- CPR certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Strong organizational, communication, and documentation skills.
- Ability to work independently in a home setting.
- Ability to travel to patients' homes and navigate varied home environments.
- Able to stand, walk, bend, lift, and carry up to 50 pounds.
- Experience with electronic health records (EHR).
- Familiarity with Medicare/Medicaid and home health documentation standards (e.g., OASIS).
- Bilingual abilities a plus.
