SUMMARY
Interviews antepartum moms to obtain Bridges survey information and refers to Social Services for additional support when warranted. Complete a Bridges Maternal Child Health Screening Tool for mothers and a Prenatal Health Screening/Social Determinants of Health Questionnaire for all pregnant women determined to be at moderate or high-risk threshold, through the Bridges Maternal Child Health Prescreen and by following the Bedside Screening Procedures, to identify pregnant women/ mothers and/or infants who might benefit from Home Visitation services.
Offer prenatal education and bedside education including, but not limited to: community baby showers, prenatal classes, connection to prenatal care, fatherhood programing, breastfeeding education, intervention and support. Support the Family Wellness Plan integration (i.e. workflow); attend Family Support Taskforce including, but not limited to any work groups related to the hospital coordination and implementation Family Wellness Plans. Make referrals to Bridges Home Visitation Service Providers, and/or other community organizations, as appropriate, based upon the results of the Prenatal or Bridges Screening Tool Questionnaire.
Provides clerical support, while maintaining exceptional standards of customer service.
- High School or equivalent
- Previous clerical experience
- Current BLS (Health Care Provider)
