Clinical Supervisor – Nursing

In this leadership role, reporting the Director of Nursing, you will be responsible for ensuring that patient care is coordinated and managed appropriately by a team of Registered Nurse (RN) Case Managers and LVNs. You will serve as the first point of escalation ensuring support is easily accessible in a timely fashion. You will participate in the identification, development, design, and implementation of nursing educational programs to support clinical excellence, determine success measures, and evaluate the effectiveness of these programs to promote ongoing staff development as well as patient and customer satisfaction.

You will also meet regularly with our customer to maintain relationships and provide updates such as readmission reports, offering insights to improve patient outcomes and reduce readmissions.

Hybrid Work Model:

  • San Jose Office: Spend 2 days per week in the office for administrative and team management duties.
  • Fieldwork: Spend 3 days per week (average 12+ per week) in the field conducting supervisory visits and offering direct support to RN Case Managers.
What you'll do as the Clinical Supervisor - Nursing
  • In‑field oversight of clinicians who provide care and treatment to patients in their place of residence.
  • Handle incoming calls timely, and follow up as necessary to ensure appropriate actions are taken to meet internal and external customer needs.
  • Coordinate training and educational programs for nurses within the field to improve clinical outcomes and competencies.
  • Assess staff learning needs and formulate specific plans and goals for the next evaluation period; perform administrative call duties; serve as a role model and resource for staff.
  • Act as the first point of contact for field clinicians to assist with patient care escalations.
  • Coordinate at least bi‑weekly case conferences with the clinical team to discuss patient progress and revise care plans.
  • Review and evaluate each case, including services provided by clinicians, conferences, record review, discussions, and verification of impressions; instruct and guide clinicians to promote effective performance and quality home‑care delivery; be available during operating hours to assist clinicians as appropriate.
  • Review patient’s clinical diagnosis, medications, procedures, and clinical course.
  • Assist clinicians in establishing immediate and long‑term therapeutic goals, setting priorities, and developing a plan of care.
