Nurse Practitioner - Telehealth

Korzen Health is partnering with a leader in the tele whole-person care model to bring integrated medical and behavioral-health support to patients with complex, chronic needs. We are seeking a part-time Nurse Practitioner to serve as the medical clinician within a collaborative, multidisciplinary care team.

This is a primarily remote role built around a team-based model. It is not a traditional full primary-care panel position. Licensed clinical social workers (LCSWs) and other care-team staff handle much of the ongoing monitoring, patient outreach, between-visit support, documentation, and care coordination. This lets you focus on work that requires an advanced-practice medical clinician.

• Conduct initial telehealth medical evaluations and periodic follow-up visits
• Review medical history, medications, chronic conditions, and care plans
• Manage common chronic medical conditions within your scope of practice
• Provide a limited amount of general primary-care support
• Prescribe medications and order labs, tests, or referrals when clinically appropriate
• Review clinically significant findings identified through ongoing patient monitoring
• Respond to issues escalated by the care team
• Provide medical input and oversight for patient care plans
• Coordinate with behavioral-health clinicians, primary-care providers, specialists, and other care team members as needed

Required

• Active New Mexico Nurse Practitioner license
• Comfort managing common chronic medical conditions
• Comfort working with behavioral-health populations
• Strong interest in collaborative, multidisciplinary care
• Ability to work alongside LCSWs and care-management staff rather than as a traditional standalone primary-care provider

Preferred

• Experience with telehealth and/or care management

• Flexible, primarily remote schedule
• A supportive, team-based model where care coordination and monitoring are shared
• Competitive hourly compensation of $55-$75, depending on experience and fit
