Medical Social Worker​/Social Worker Assistant-Southern San Luis Obispo

Central Coast Home Health & Hospice is a fast-growing, locally owned and operated organization with an excellent reputation throughout the Central Coast. We are now seeking a full-time
Medical Social Worker or Social Worker Assistant to work with both Home Health and Adult Hospice patients in the Southern San Luis Obispo areas (5 cities
- Lompoc).

This role offers the opportunity to make a meaningful impact while enjoying autonomy and flexibility, supported by clinical supervision from an experienced LCSW. Clinical supervision is available for candidate working toward licensure.

• Bachelor's Degree in Sociology, Psychology, Human Development
• Master's Degree from a school accredited by the Council on Social Work Education

• Minimum of one-year social work experience in a healthcare setting, home health &/or hospice experience preferred.
• Understand the "hospice philosophy" and issues of death and dying
• Reliable vehicle to drive for work
• Current Driver's license
• Clean background & driving record
• Maintain auto insurance coverage
• CPR certified

• Act calmly and professionally at all times
• Strong organization and communication skills
• Work well independently and in a team environment
• High attention to detail and good follow up
