Under supervision, you will perform a range of functions, covering multiple pharmacy locations, units or other settings. Your duties include the preparation and dispensing of drugs in all inpatient settings; review and verification of medication orders and patient medical records; optimizing safe medication therapy; review of patient medication profiles; counseling patients on appropriate drug use; interacting and/or rounding with other health professionals to help provide safe and effective use of medications;

provision of advanced clinical services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, TPN and aminoglycoside monitoring, pharmacist interventions , anticoagulation ) to neonatal, pediatric and adult patients according to departmental policy and procedures, with possible focus on pharmacy specialty (i.e., oncology, pediatrics, geriatrics, ambulatory care); oversee dispensing, return and waste of controlled substances on assigned shift/unit.

Initiate, innovate and complete clinical, operational and/or management projects, in collaboration with Pharmacy Management. You will work in assigned centralized and decentralized Pharmacy areas and patient care units.

Schedule:

overnight shifts - 7 days on, 7 days off (includes weekends and holidays)

Hourly Salary Range: $90.64 - $112.68 (Union represented, step based salary depending on years of experience)

We’re seeking a dedicated clinical professional with:

• Pharm
  
  D. degree and current CA Pharmacy license
• ASHP-accredited residency (preferred)
• 2 years inpatient pharmacy experience (preferred)
• Exceptional communication, interpersonal, organizational and follow-through skills.
• Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations.
• Ability to compound intravenous admixtures and other sterile products.
