Per Diem, 8-hour Day shifts.

Case Management is a collaborative practice that includes patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. The process facilitates communication and care coordination across the continuum through effective transitional care management. The goal is optimal health, access to services, advocacy, appropriate utilization of resources and collaboration with post‑acute care providers to ensure patients’ needs are met in the community.

The Care Manager utilizes assessment, planning/intervention, implementation, care coordination, monitoring, evaluation of the plan of care and communication to meet individual healthcare needs.

• Conduct comprehensive patient assessments and develop individualized care plans.
• Coordinate with multidisciplinary team members and community resources.
• Monitor patient progress, evaluate outcomes, and adjust care plans as needed.
• Facilitate transitional care management and post‑acute transitions.
• Advocate for patients and ensure appropriate utilization of services.

• Bachelor’s Degree
• California Registered Nurse License upon hire
• 2 years of experience in a healthcare related field (Acute, Ambulatory, or Post‑Acute).
• 2 years of experience in Acute Care Management (Care Coordination or Utilization Management) or completion of the Transitions in Practice (TIP) program for Care Managers.

• Master’s Degree in a health‑related or science field

Base pay $56.44 – $87.63 per hour. Additional compensation such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives and bonus opportunities may be available.
