Upward Bound House (UBH) is a mission‑driven nonprofit ending intergenerational family homelessness. For over 30 years, we’ve delivered a continuum of care—from crisis to permanent housing—rooted in empowerment, collaboration, and trauma‑informed practices.

About

The Role

Provide trauma‑informed, strengths‑based case management and housing navigation to families experiencing homelessness. Manage a caseload of 21 households and support each to secure permanent housing within 120 days of program entry.

What You’ll Do
  • Create individualized housing plans; identify and reduce barriers to stability.
  • Ensure clients are document‑ready (, SSN, income, vital records).
  • Lead housing search: landlord outreach, applications, and subsidy navigation.
  • Maintain accurate, timely HMIS data; meet all contract/agency standards.
  • Conduct regular check‑ins, assessments, case notes, and progress updates.
  • Coordinate referrals (mental health, employment, childcare, benefits).
  • Provide crisis intervention and conflict resolution as needed.
  • Collaborate in team meetings, case conferences, and trainings.
What You Bring
  • 2+ years in housing/homeless services; interim/residential experience preferred.
  • Working knowledge of housing programs, landlord engagement, and rapid rehousing.
  • Proficiency with HMIS data entry/reporting.
  • Commitment to trauma‑informed, client‑centered practice with diverse populations.
  • Strong organization, documentation, and communication skills.
  • Valid driver’s license, reliable vehicle, and current insurance/registration.
  • Proven ability to work independently and thrive in a collaborative team.
Mandatory Requirements

Employment eligibility, reliable transportation, TB test, background check, CPR/First Aid, valid CA driver’s license, personal auto insurance, and insurability under UBH policy.

Physical/Work Environment

The role may involve contact with individuals in crisis and responding to urgent situations. UBH provides training and support. Reasonable accommodations are available; please notify HR at hire or as needs arise.
